Activated in the Reserves on 13 Jun 1952. Redesignated as 349 Tactical Hospital on 18 May 1954. Ordered to active service on 28 Oct 1962. Relieved from active duty on 28 Nov 1962. Discontinued, and inactivated on 11 Feb 1963. Redesignated as 349 USAF Clinic (Associate) on 18 Oct 1982. Activated on 1 Dec 1982. Redesignated as: 349 Medical Squadron on 1 Oct 1994; 349 Aerospace Medicine Squadron on 1 Jul 2000.
