Escudo de ouro, monte de dois cômoros de verde, movente dos flancos e de um pé ondado de prata e azul de duas tiras, carregado de ponte antiga de três arcos de prata, lavrada de negro e encimada por fonte termal de vermelho, repuxando água de azul. Coroa mural de prata de três torres. Listel branco, com a legenda a negro: «CALDAS DE VIZELA (S. JOÃO)».
